Many Older Men Won't Benefit from PSA Test

(HealthDay News) -- Routine prostate specific antigen (PSA) blood tests offer no benefit to many men ages 75 to 80, a new study suggests.

Researchers analyzed data from 727 prostate cancer patients and 122 cancer-free men who took part in the U.S. National Institute on Aging's Baltimore Longitudinal Study of Aging and had regular PSA testing.

The Johns Hopkins-led study found that none of the men between 75 and 80 years old who had PSA levels lower than 3 nanograms per milliliter died of prostate cancer and only one developed aggressive prostate cancer. However, men of all ages with a PSA level of 3 nanograms per milliliter or higher had a continually rising risk of dying from prostate cancer.

The findings, published in the April issue of the Journal of Urology, suggest that many older men may no longer require PSA testing. If confirmed in future studies, the results may help determine more specific age-based guidelines for when it's safe to stop PSA screening, said lead investigator Dr. Edward Schaeffer, an assistant professor of urology at Johns Hopkins University School of Medicine.

PSA testing helps detect prostate cancer at the early stages and is credited with helping reduce prostate cancer deaths. But discontinuing unnecessary PSA testing in certain older men could help reduce screening costs and health problems caused by additional tests or treatments.

"We need to identify where we should best focus our health care dollars by concentrating on patients who can actually benefit from PSA testing. These findings give a very strong suggestion of when we can start to counsel patients on when to stop testing," Schaeffer said.

Black Raspberries Get Cancer at the Genes

Black raspberries slow cancer by altering hundreds of genes.

New research strongly suggests that a mix of preventative agents, such as those found in concentrated black raspberries, may more effectively inhibit cancer development than single agents aimed at shutting down a particular gene.

Researchers at the Ohio State University Comprehensive Cancer Center examined the effect of freeze-dried black raspberries on genes altered by a chemical carcinogen in an animal model of esophageal cancer.

The carcinogen affected the activity of some 2,200 genes in the animals’ esophagus in only one week, but 460 of those genes were restored to normal activity in animals that consumed freeze-dried black raspberry powder as part of their diet during the exposure.

These findings, published in the journal Cancer Research, also helped identify 53 genes that may play a fundamental role in early cancer development and may therefore be important targets for chemoprevention agents.

“We have clearly shown that berries, which contain a variety of anticancer compounds, have a genome-wide effect on the expression of genes involved in cancer development,” says principal investigator Gary D. Stoner, a professor of pathology, human nutrition and medicine who studies dietary agents for the prevention of esophageal cancer.

“This suggests to us that a mixture of preventative agents, which berries provide, may more effectively prevent cancer than a single agent that targets only one or a few genes.”

Stoner notes that black raspberries have vitamins, minerals, phenols and phytosterols, many of which individually are known to prevent cancer in animals.

“Freeze drying the berries concentrates these elements about ten times, giving us a power pack of chemoprevention agents that can influence the different signaling pathways that are deregulated in cancer,” he says.

To conduct this study, Stoner and his colleagues fed rats either a normal diet or a diet containing 5 percent black-raspberry powder. During the third week, half the animals in each diet group were injected three times with a chemical carcinogen, N-nitrosomethylbenzylamine. The animals continued consuming the diets during the week of carcinogen treatment.

After the third week, the researchers examined the animals’ esophageal tissue, thereby capturing gene changes that occur early during carcinogen exposure. Their analyses included measuring the activity, or expression levels, of 41,000 genes. In the carcinogen-treated animals, 2,261 of these genes showed changes in activity of 50 percent or higher.

“These changes in gene expression correlated with changes in the tissue that included greater cell proliferation, marked inflammation, and increased apoptosis,” Stoner says.

In the animals fed berry powder, however, a fifth of the carcinogen affected genes – exactly 462 of them – showed near-normal levels of activity, when compared with controls. Most of these genes are associated with cell proliferation and death, cell attachment and movement, the growth of new blood vessels and other processes that contribute to cancer development. The tissue also appeared more normal and healthy.

Lastly, of the 462 genes restored to normal by the berries, 53 of them were also returned to normal by a second chemoprevention agent tested during a companion study.

“Because both berries and the second agent maintain near-normal levels of expression of these 53 genes, we believe their early deregulation may be especially important in the development of esophageal cancer,” Stoner says.

“What’s emerging from studies in cancer chemoprevention is that using single compounds alone is not enough,” Stoner says. “And berries are not enough. We never get 100 percent tumor inhibition with berries. So we need to think about another food that we can add to them that will boost the chemopreventive activities of berries alone.”

Father's Age a Factor in Infertility

(HealthDay News) -- Among couples with fertility problems, those in which the man is over the age 35 have lower pregnancy rates and increased chances of miscarriage, a new study shows.

To come to this conclusion, French researchers looked at more than 12,000 couples who went to a fertility clinic in France.

In most of the cases, the couples were being treated due to the man's infertility. The couples underwent a total of 21,239 intrauterine inseminations (IUIs).

The researchers found that women over age 35 had a pregnancy rate of 8.9 percent, compared to 14.5 percent in younger women.

"But we also found that the age of the father was important in pregnancy rates -- men over 35 had a negative effect. And, perhaps more surprisingly, miscarriage rates increased where the father was over 35," study author Dr. Stephanie Belloc, of the Eylau Center for Assisted Reproduction in Paris, said in a prepared statement.

This is the first study to document such a strong paternal effect on reproductive outcomes. The findings were presented Monday at the European Society of Human Reproduction and Embryology annual conference, in Barcelona.

"How DNA damage in older men translates into clinical practice has not been shown up to now. Our research proves for the first time that there is a strong paternal age-related effect on IUI outcomes, and this information should be considered by both doctors and patients in assisted reproduction outcomes," Belloc said.

"We believe that the use of IVF or ICSI should be suggested to infertile patients where either party is over 35 years of age," she added.

What is Fucoxanthin?

Fucoxanthin is a type of carotenoid found naturally in edible brown seaweed such as wakame (Undaria pinnatifida) and hijiki (Hijikia fusiformis), which are used widely in Asian cuisine. Wakame is the seaweed used in miso soup.

Fucoxanthin is also found in much smaller amounts in red seaweed (the kind typically used in Japanese sushi rolls) and green seaweed.

Both wakame and hijiki are available at Japanese specialty food stores, some health food stores and online. Although brown seaweed is the richest source of fucoxanthin, you would have to eat an unrealistic amount of it daily to get fucoxanthin levels close to those used in research studies.

Fucoxanthin is also available as a nutritional supplement in capsule form and can be found in some health food stores and online.

Why Do People Use Fucoxanthin?
Weight Loss
Fucoxanthin is being explored for weight loss.

So far, only animal studies have been done. Japanese researchers have found that fucoxanthin (isolated from wakame) promotes the loss of abdominal fat in obese mice and rats. Animals lost five to 10% of their body weight.

Although it's not fully understood how fucoxanthin works, it appears to target a protein called UCP1 that increases the rate at which abdominal fat is burned. Abdominal fat, also called white adipose tissue, is the kind of fat that surrounds our organs and is linked to heart disease and diabetes. Fucoxanthin also appears to stimulate the production of DHA, one of the omega-3 fatty acids found in fatty fish such as salmon.

Although it's promising and already a popular nutritional supplement, more research is needed to determine if fucoxanthin will work in the same way in humans. If it does prove to be effective, fucoxanthin could be developed into a diet pill for obesity.

Diabetes

Fucoxanthin has also been found in animal studies to decrease insulin and blood glucose levels. Researchers hypothesize that fucoxanthin anti-diabetes effect may be because fucoxanthin appears to promote the formation of DHA (the omega-3 fatty acid found in fish oil). DHA is thought to increase insulin sensitivity, improve triglycerides and reduce LDL ("bad") cholesterol.

Cancer

Preliminary research in test tubes suggests that fucoxanthin may have anti-tumor effects. No studies have looked at whether this holds true in humans or if taken orally. It's far too early for fucoxanthin to be used as a complementary treatment for cancer.

Side Effects
Because there hasn't been research on fucoxanthin in humans, the possible side effects aren't known.

People shouldn't consume large amounts of wakame or other types of seaweed as a source of fucoxanthin. Seaweed is rich in iodine and excessive consumption may result in iodine poisoning. High levels of iodine can interfere with the function of the thyroid gland. Also, consuming excess amounts of iodine-rich foods isn't recommended if there is a known allergy or hypersensitivity to iodine.

Sticking With My Morning Workout

To make running a daily habit, I’m setting concrete goals

I never thought I’d say this, but my long weekend runs have become the easiest part of my training program. Sure, it took me 98 minutes to run 9.5 miles on Saturday, but 98 measly minutes out of a full weekend of leisure is an easy sacrifice.

Trying to run three to five miles three days a week, on the other hand, is a different story. With long work days, dinner dates, and meet-ups with friends, I’ve been having trouble squeezing it in.

During the winter, I had no choice but to use the treadmill; that meant I could run on my lunch break or right after work. Now that the weather’s nicer, I’ve been spoiled—and suddenly, it just seems wrong (and really, really difficult) to run in the sticky, sweaty gym.

Running in midtown Manhattan isn’t an option I want to consider either, thanks to the crowded sidewalks and vehicle exhaust. And for safety reasons, I don't particularly like running anywhere after dark. So that means I have to either cut my day short and hightail it home to Brooklyn at a decent hour, or do the unthinkable: Wake up early.

I am not a morning person. But for the last two weeks, I’ve been trying desperately to haul my butt out of bed for a sunrise workout. I've marked my calendar. I've programmed “Gonna Fly Now” as my new cell phone alarm. I've asked my boyfriend to call me at 6 a.m. and scold me for being lazy. A few ambitious mornings I’ve made it out, but nothing’s been able to consistently beat out the allure of the snooze button.

I posed my dilemma to Walter R. Thompson, PhD, professor of kinesiology and health at Georgia State University and an expert in exercise adherence. Give it some time, he advised: It will probably take about a month of regular morning workouts before I start to notice their benefits and actually look forward to them. A big part of this, he explained, is breaking old routines and establishing new ones.

“You’ve spent the last 25 years getting out of bed and not even thinking about exercise,” he told me. “That’s a hard habit to break.”But my Monday-Wednesday-Thursday schedule is a good start, he added. “Sleeping in once or twice a week gives you a breather to look forward to, but you’re still exercising enough to meet your goals.”

Speaking of goals, he suggested setting both short-term (to keep from losing focus) and long-term ones (to remember the progress I've made, even in the event of setbacks along the way).

So for the remainder of my training, I’m doing just that: My long-term goal is to run the entire Brooklyn Half Marathon on May 3. And next week, maybe I’ll aim to shave another minute off my five-mile time.

This week? I’m just focusing on getting up and out the door when I say I will.

The "Real" Weight-Loss Numbers

I have been concerned for some time that my measurements are off. Although I am using an expensive and technically advanced digital scale to measure weight loss, there's little consistency in the readings it gives.
