Green Tea Antioxidant May Help Prevent Alzheimer's

(HealthDay News) -- An antioxidant found in green tea appears to prevent the development of amyloid fibrils, a toxic protein associated with Alzheimer's and Parkinson's disease, a new study finds.

Amyloid plaque is commonly seen in the brains of Alzheimer's patients and appears to disrupt the function of cells. Strategies to prevent the development of amyloid plaque are one avenue being explored in the prevention and treatment of Alzheimer's.

Now, a German team says the tea antioxidant, called epigallocatechin gallate (EGCG), has potent anti-plaque ability.

"We can use small molecules like EGCG to convert certain misfolded structures of a protein into a new type of molecule, which is less toxic or not toxic for cells," said lead researcher Erich Wanker, from the Max Delbrueck Center for Molecular Medicine in Berlin.

The findings are published in the May 30 online edition of Nature Structural & Molecular Biology.

The accumulation of amyloid plaque in Alzheimer's and other neurodegenerative diseases, such as Parkinson's, are thought to be caused by the misfolding of certain proteins, which then become toxic to cells. The way proteins fold is key to their function, the researchers explained.

In experiments in the laboratory, the German team found that EGCG seems to change potentially harmful proteins into proteins that are not detrimental to brain cells. "We are able to convert a toxic structure into a less toxic structure," Wanker explained.

Because EGCG binds to unfolded proteins -- which are not associated with Alzheimer's -- the discovery could lead to medications that recognize the more troublesome proteins and convert them to harmless substances.

"This method could be more generally used to get rid of or remove the concentration of misfolded proteins in cells," Wanker said. "This strategy should be tested with patients. If treated early on, it could prevent the formation of amyloid plaque," he speculated.

Whether this type of treatment could reverse plaques that have already formed in the brain isn't known, Wanker said.

He noted that the study remains basic science, and he was cautious about recommending green tea as a way of preventing Alzheimer's disease. "I don't want to do a lot of speculating which could point people in the direction that could be harmful," Wanker said. "We have to go step-by-step."

One expert believes the approach could yield real results, however.

"Red wine, yellow curry and green tea have suspected health benefits because of high content of antioxidants," said Greg M. Cole, a neuroscientist at the Greater Los Angeles VA Healthcare System, and associate director of the Alzheimer's Disease Research Center at UCLA David Geffen School of Medicine. He was not involved in the study.

"This study provides evidence that a compound called EGCG, one of the major polyphenols in green tea, may be useful for diseases like Parkinson's and Alzheimer's, because it can block the formation of the filament-forming protein aggregates implicated in causing disease," Cole said.

One novel aspect of the study is the authors' demonstration that EGCG prevents toxic filament formation by redirecting the aggregating proteins to make non-toxic proteins, Cole said.

"This is surprising, because similar protein aggregate spheres called amyloid oligomers can be highly toxic to neurons and synapses," Cole said. "It will be important for the authors to prove that the EGCG-directed proteins also lack toxicity to synapses which were not present in the systems used to test toxicity," he said.

Assuming that the green tea compound has a stable effect and chronically blocks toxicity to real neurons and synapses, it could have genuine potential for Alzheimer's patients, Cole said.

"The major caveat is the very poor absorption and delivery of EGCG seen in some studies," Cole said. The fact that EGCG isn't available for patenting by pharmaceutical companies might be a problem, too, he said, since it could "limit the investment needed for clinical trials of sufficient size to prove that it really works."

In related research, a team of American scientists said that interrupting a key signaling pathway in immune system cells allowed those cells to enter the brain and attack and remove amyloid plaque.

Reporting May 30 in Nature Medicine, a team led by research scientist Terrence Town, of Cedars-Sinai Medical Center, Los Angeles, conducted their study in genetically engineered mice. The group blocked a molecule that typically suppresses a portion of the immune response. Once the system was freed up, immune cells called macrophages made their way to the brains and devoured up to 90 percent of amyloid plaques, the team said.

"If these experimental animals are representative of the clinical syndrome of Alzheimer's disease, we may have a therapeutic target that we did not have before," study co-author Dr. Jun Tan, of the University of South Florida, said in a statement.

Blackberries, Broccoli Sprouts Battle Cancer

(HealthDay News) -- Your local farmer's market might hold the key to cancer prevention, since new research shows that black raspberries, broccoli sprouts and some raw vegetables reduce the risk of esophageal and bladder cancers.

Data from three studies on the subject was presented Thursday at the American Association for Cancer Research's Sixth Annual International Conference on Frontiers in Cancer Prevention, in Philadelphia.

Fruits and vegetables have long been known to help reduce the risk of certain cancers. Based on prior research, the American Cancer Society recommends eating five servings of fruits and vegetables daily.

In the first study, Ohio State University researchers found black raspberries may protect against esophageal cancer by reducing the oxidative stress that results from Barrett's esophagus, a precancerous condition usually caused by gastroesophageal reflux disease. The esophagus is a long tube that connects the throat to the stomach. Reflux disease causes stomach acid to continually splash back up into the esophagus.

"Specifically in the case of Barrett's patients, reflux of the stomach and bile acid contribute to ongoing oxidative damage. Thus, our hypothesis is that feeding a food that is high in potential protective constituents, such as antioxidants, vitamins, minerals and other phytochemicals, may help restore the oxidative balance," lead researcher Laura Kresty said.

People with Barrett's esophagus typically are 30 to 40 times more likely to develop esophageal cancer, which has a poor five-year survival rate of 15 percent.

The team gave 32 grams to 45 grams of black raspberries daily for six months to 20 patients with Barrett's esophagus. They analyzed changes in blood, urine and tissue before, during and after the treatment, and found lower levels of some of the chemical markers of oxidative stress in both urine and tissue samples.

Black raspberries previously have been shown to reduce the risk of oral, esophageal and colon cancer in animal models, according to the researchers, who called for further study in humans.

Dietitian Wendy Demark-Wahnefried, a professor of behavioral science at M.D. Anderson Cancer Center at the University of Texas in Houston, said she would feel comfortable advising people with Barrett's to eat black raspberries. "It couldn't hurt," she said, but added that further studies need to find out if the berries really do prevent cancer.

In other research presented at the meeting, broccoli sprouts and cruciferous vegetables both showed promise in the fight against bladder cancer, according to two separate teams from the Roswell Park Cancer Institute in Buffalo, N.Y.

Using a rat model, a team lead by Dr. Yuesheng Zhang, a professor of oncology, demonstrated that a broccoli sprout extract reduced bladder cancer in rats by 70 percent.

"Our present study shows that broccoli sprout extracts fed to rats in the diet inhibits bladder cancer development induced by a carcinogen. We don't yet know if the extracts inhibit the growth of a existing bladder cancer," said Zhang, who explained that broccoli sprouts are a rich source of a well-known cancer preventive agent known as sulforaphane.

"We next plan to find out if broccoli sprout extracts can fight bladder cancer in humans," Zhang noted.

A second team at the institute found that people who ate three or more servings of raw, cruciferous vegetables per month reduced their risk of bladder cancer by 40 percent.

Cruciferous vegetables include broccoli, cabbage and cauliflower.

The team analyzed the dietary habits of 275 people with early bladder cancer and 825 people who were cancer-free. The researchers specifically asked how many servings of raw or cooked cruciferous vegetables they ate before their diagnosis and whether they smoked.

Analysis of the data showed that the more raw, cruciferous vegetables people ate, the lower their risk of bladder cancer. In comparison to people who smoked and ate fewer than three servings of raw vegetables a day, nonsmokers eating at least three servings of cruciferous vegetables daily were 73 percent less likely to develop bladder cancer.

"In our study, we do find intake of raw cruciferous vegetables showed risk reduction of bladder cancer in smokers, and even the heavier smokers," said lead researcher Li Tang.

The researchers stressed that the benefits are derived from raw cruciferous vegetables, giving cole slaw the edge over cabbage soup when it comes to cancer prevention.

"This confirms that there are a variety of compounds within fruits and vegetables that contribute to reducing the risk of cancer. Research like these studies contribute to our knowledge about what the impact of specific nutrients may be on specific types of cancer," said Colleen Doyle, director of nutrition and physical activity for the American Cancer Society.

"Cooking leaches out some nutrients but makes others more absorbable. Until we know more in this regard, the bottom-line message for consumers is eat at least five servings of fruits and vegetables each day, raw and/or lightly cooked. Focus on those with the most color, since, in general, fruit and vegetables with the most color have the most cancer-fighting antioxidants and phytochemicals."

"Surveys we've done indicate many people don't think they have control over their cancer risk, but studies clearly indicate they do. For the majority of people who don't smoke, watching their weight, being more active and eating a healthy diet are the most important ways to reduce cancer risk," Doyle said.

Advantages and Disadvantages of Alcohol Intake on Cardiovascular Health Reviewed

The latest American Heart Association guidelines caution people not to start drinking alcohol if they do not already drink alcohol, according to a review of the advantages and disadvantages of alcohol intake on cardiovascular health published in the August 23 Online First issue of the Journal of the American College of Cardiology.

"Accumulating scientific evidence indicates that light to moderate drinking done on a daily basis may significantly reduce the risks of coronary heart disease (CHD) and all-cause mortality," write James H. O'Keefe, MD, FACC, and colleagues from the Mid America Heart Institute in Kansas City, Missouri. "In contrast, excessive alcohol intake and binge drinking are toxic to both the heart and overall health and are the third leading cause of premature death among Americans.

The purpose of the present review is to:
1) outline the specific benefits and risks of alcohol, and the threshold of intake at which drinking becomes a health danger rather than an advantage;
2) detail the mechanisms whereby alcohol confers cardioprotection; and
3) discuss the ideal quantities, drinking patterns, and beverages, and which individuals are most likely to benefit."

Research to date has shown J-shaped relationships between alcohol consumption and several adverse health outcomes, including all-cause mortality, CHD, diabetes, hypertension, congestive heart failure, stroke, dementia, and Raynaud's phenomenon.

Some cardioprotective benefits have been demonstrated for light to moderate alcohol consumption (up to 1 drink daily for women and 1 or 2 drinks daily for men). Most studies have shown that light to moderate drinking is associated with risk reductions for CHD of approximately 30% to 35%.

Alcohol consumed in moderation seems to have an antiatherosclerotic effect, with decreased incidence of peripheral arterial disease and decreased atherosclerotic burden shown by coronary angiography, computerized tomography-detected coronary calcium, and carotid ultrasound.
Like exercise, alcohol consumption seems to be most cardioprotective when done daily and in moderation. However, increasingly excessive consumption is associated with proportional worsening of outcomes.

Because of the beneficial effects on high-density lipoprotein (HDL) cholesterol, insulin action, and inflammation, light to moderate alcohol intake may be particularly helpful for patients with abnormal glucose metabolism and/or insulin resistance. Light to moderate alcohol intake may be associated with reductions in the prevalence and incidence of diabetes, and a large meta-analysis with 12-year follow-up showed a 30% reduction in new diabetes in those who consumed 1 to 2 drinks daily.

The cardiovascular benefits associated with alcohol consumption protection are thought to be mediated by improvements in insulin sensitivity and HDL cholesterol. The major protective component seems to be the ethanol itself, and not the other ingredients found in different types of alcoholic beverages.

In a meta-analysis incorporating data from more than 1 million participants, consumption of 1 drink daily by women and 1 or 2 drinks daily by men was associated with an 18% reduction in total mortality, but daily intakes of more than 2 drinks in women and 3 drinks in men were associated with dose-dependent increased mortality.

Although low-dose, daily alcohol consumption has been linked to better health than less frequent use, binge drinking increases cardiovascular events and mortality, even in otherwise light drinkers.

"Alcohol should not be universally prescribed for health enhancement to nondrinking individuals owing to the lack of randomized outcome data and the potential for problem drinking," the study authors note. "Alcohol (ethanol) consumption is analogous to the proverbial double-edged sword, and perhaps no other factor in cardiovascular (CV) health is capable of cutting so deeply in either direction depending on how it is used."

Guidelines from the United Kingdom suggest that middle-aged or elderly men and postmenopausal women who drink seldom or never might consider the possibility that light drinking would benefit their health. On occasion, the reviewers have made a similar recommendation to patients who do not smoke and who have no personal or family history of substance abuse and no history of depression or bipolar disorder. However, they argue that light to moderate drinking should not be universally recommended to the general public or even to patients with cardiovascular disease.

As an analogy, they point out that observational data and findings from randomized trials using surrogate endpoints suggested that hormone replacement therapy for women and antioxidant vitamins improved cardiovascular outcomes, whereas later randomized trials with larger samples led to the opposite conclusions. They note that no randomized trials of alcohol for improving clinical outcomes have yet been performed. In observational studies, residual unmeasured confounding factors could be contributing to apparent benefits that seem to be associated with light to moderate drinking.

The investigators further note that heavy drinking has caused significant individual and societal problems and morbidity. In the past 15 years, the rates of alcohol abuse and binge drinking have been increasing, according to the findings of some studies. Alcohol abuse causes 100,000 deaths each year in the United States, and it has been deemed the third largest preventable cause of death.

Excessive alcohol consumption has been associated with increased risks for all-cause mortality, motor vehicle crashes, stroke, cardiomyopathy, cardiac dysrhythmia, sudden cardiac arrest, suicide, cancer (particularly of the breast and gastrointestinal tract), cirrhosis, fetal alcohol syndrome and sleep apnea.

"Sobering statistics warn that moderate daily drinking is a slippery slope that many individuals cannot safely navigate," the study authors conclude. "The latest American Heart Association guidelines caution people not to start drinking if they do not already drink alcohol, because it is not possible to predict in which people alcohol abuse will become a problem. Until we have more randomized outcome data, and tools for predicting susceptibility to problem drinking, it would seem prudent to encourage physicians and patients to focus on more innocuous interventions to prevent CHD."

Resolve to Exercise Your Brain

(HealthDay News) -- Getting in shape tops many New Year's resolution lists, but the Alliance for Aging Research is encouraging Americans to boost their brain health, too.

"There is a lot we can do to keep our brains healthy and potentially prevent or lessen the cognitive decline that often comes with aging," Daniel Perry, executive director of the nonprofit Alliance for Aging Research, said in a prepared statement. "We are encouraging people to take steps to improve brain health as part of their overall fitness regimen for the New Year."

The Alliance for Aging Research recommends these 10 steps for improving your brain health.

Eat a Brain-Healthy Diet. A diet rich in omega-3 fatty acids (commonly found in fish), protein, antioxidants, fruits and vegetables and vitamin B; low in trans fats; and with an appropriate level of carbohydrates will help keep your brain healthy.

Stay Mentally Active. Activities such as learning a new skill or language, working on crossword puzzles, taking classes, and learning how to dance can help challenge and maintain your mental functioning.

Exercise Regularly. Exercising often can increase circulation, improve coordination, and help prevent conditions that increase the risk of dementia such as heart disease, stroke and diabetes.
Stay Social. Spending time with friends, volunteering, and traveling can keep your mind active and healthy.

Get Plenty of Sleep. Not getting enough sleep can have a negative impact on brain health.

Manage Stress. Participating in yoga, spending time with friends, or doing other stress-relieving activities can help preserve your ability to remember and learn.

Prevent Brain Injury. Wearing protective head gear and seat belts can help you avoid head injury, which has been associated with an increased risk of dementia.

Control Other Health Conditions. Maintaining a healthy weight, exercising, eating a well-balanced and nutritious diet, and controlling stress can help reduce your risk of diseases that affect your brain, including diabetes, heart disease, high blood pressure and hypertension.

Avoid Unhealthy Habits. Smoking, heavy drinking and use of recreational drugs can increase the risk of dementia and cognitive decline.

Consider Your Genes. If your family history puts you at risk for developing dementia, work with your doctor to find ways to maintain your brain health to help avoid or slow the progression of cognitive decline.

Q: What do you make of the new study showing that the test for prostate cancer may miss some disease? How can men best protect themselves? -- Robert P.

A: Results of a study published in the May 27, 2004, issue of the New England Journal of Medicine showed that the blood test for prostate cancer misses about 15 percent of all cases, some of which are aggressive enough to be life-threatening. The test looks for a protein (prostate specific antigen, known as PSA) released by prostate cells when cancer is present and as the prostate enlarges, which it does as men age. Until results of the study were published, a PSA count under four meant that a man was disease-free; the higher the PSA count, the more likely the presence of cancer. The only reliable way to currently confirm or rule out cancer is with a biopsy.

Now that the study has found that some men with a PSA of "four" or lower may still have cancer, doctors don't know what to recommend. To be safe, some specialists say they'll suggest biopsies to all men with a PSA above 2.5; others think there already are too many biopsies, too much testing and needless treatment of prostate cancers that might never become life-threatening. While prostate cancer can be very aggressive, most tumors grow slowly and the risk of death is low -- only three percent. What we need is a better test to distinguish between slow growing tumors that may not require treatment and fast-growing cancer that demands an aggressive approach.

Several methods have been suggested to improve the testing methods for prostate cancer and thus reduce the number of unnecessary biopsies. Adjusting the expected results for the age of the patient and size of the prostate as well as measuring PSA changes over time are some possibilities.

Still, decisions on how to best handle early prostate cancer will remain clouded until better tests are developed and perfected.

In the meantime, make sure that you and your doctor are aware of your personal risks. The incidence of prostate cancer increases with age; the disease occurs most frequently among African-American men, those with a family history of the disease, and those whose diets are high in animal fat.

To reduce your risk, include more watermelon, pink grapefruit, and cooked tomatoes in your diet. All contain the prostate protective antioxidant lycopene. I also recommend drinking lots of water (dehydration stresses the prostate), adding soy foods to your diet and eating less fat, particularly saturated fats found in red meat as well as hydrogenated oils, processed foods and other sources of trans fats. (Be sure to eat the right kinds of fat such as olive oil, avocados and nuts.)

Q: What do you think of the recent news that chocolate is good for the heart? -- Charlie R. A: The latest news about chocolate -- that it makes blood vessels more flexible -- adds to accumulating evidence that chocolate offers a number of health benefits and may be good for the heart. Earlier findings had shown that chocolate contains polyphenols, the same kinds of antioxidants found in red wine and green tea; stearic acid, a type of fat that doesn't raise cholesterol levels; and flavonoids, which reduce the stickiness of platelets, inhibiting blood clotting and reducing the danger of coronary artery blockages.
In the most recent study, researchers at Athens Medical School in Greece recruited 17 healthy young volunteers who agreed to eat a 3.5-ounce bar of dark chocolate and then undergo ultrasound tests to see how the chocolate affected the functioning of endothelial cells in blood-vessel walls. Normally, these cells control the stiffness of blood vessels by secreting substances that regulate flexibility.
The researchers found that on the days the volunteers ate dark chocolate (as opposed to those who ate fake chocolate), endothelial function was improved for about three hours. These are interesting findings, but we don't yet know what they mean in terms of preventing cardiovascular disease, heart attacks, or deaths. This study doesn't tell us whether eating dark chocolate on a regular basis would reduce the risk of heart disease.
We do know, however, that eating too much chocolate can lead to weight gain, which might cancel out any beneficial effects that chocolate confers. So the good news about chocolate isn't a license to overindulge. I enjoy high-quality dark chocolate from France, Belgium and Venezuela. It is so richly delicious that a single piece is a satisfying snack or after-dinner treat. Limit yourself to an ounce several times a week. If you can't find good imported chocolate, look for a domestic brand that contains at least 70 percent cocoa.

Q: I was recently diagnosed with multiple sclerosis and have been taking chlorophyll supplements to boost my immune system. Is this wise, or should I be more cautious about chlorophyll? -- Linda F.

A: Chlorophyll, the green pigment that gives plants their color, has no function in the human body. That fact hasn't stopped marketers from promoting supplements containing chlorophyll and suggesting that it can benefit patients with conditions ranging from cancer to arthritis to multiple sclerosis (MS). Chlorophyll can't hurt, but it isn't an immune booster, and I don't know of any research suggesting that it helps patients with MS.

Unfortunately, we know little about what causes MS and what factors influence its progression and outcome. It begins with localized inflammatory damage to the myelin sheaths surrounding nerve fibers due to an attack by the immune system. This interferes with nerve impulses and can lead to symptoms such as muscle weakness, loss of vision, and a variety of other impairments.

A drug called beta-interferon has become the conventional treatment for MS patients. While it can slow the progression of the disease, it is expensive and produces unpleasant side effects. Whether or not you take beta-interferon, you can try to influence the course of the disease with the stress reduction, mind/body treatments and lifestyle changes recommended below:
  • Decrease protein intake toward 10 percent of daily calories, and emphasize plant rather than animal protein.
  • Eliminate milk and milk products, substituting other calcium sources.
  • Eat organically grown fruits and vegetables as well as organic products made from wheat and soy.
  • Eliminate polyunsaturated vegetable oils, margarine, vegetable shortening, all partially hydrogenated oils, foods (such as deep-fried foods) that might contain trans-fatty acids.
  • Use extra-virgin olive oil as your main fat.
  • Increase intake of omega-3 fatty acids from fish, walnuts, or flax and hemp seeds.
  • Eat more fruits and vegetables, preferably organic.
  • Eat ginger. Turmeric can also be helpful. (I recommend the product called Turmeric Force from New Chapter.)
  • Take acidophilus culture and psyllium if constipation is a problem, or use the ayurvedic herbal bowel regulator, triphala.
  • Take my antioxidant and daily multivitamin formula plus an additional B-50 complex vitamin daily.
  • Take 5 grams of soy lecithin granules daily (store in the refrigerator).
  • Take 30 milligrams of Coenzyme Q10 (CoQ-10) two or three times a day.
  • Do some kind of light aerobic exercise on a regular basis. Choose something you enjoy; don't push yourself to the point of exhaustion.
  • Try visualization, meditation, and hypnotherapy to redirect your mental energies in positive directions.
  • Experiment with traditional Chinese medicine and Ayurvedic medicine from qualified practitioners.
  • Ashwaganda, an Ayurvedic herb may be helpful. Finally, talk to a Chinese medical practitioner about the use of bee sting therapy, which has helped in many cases.

Q: I just bought some supplements and the clerk at the health food store told me I needed to take 6,000 mg of vitamin C per day. That sounds like an awful lot. What do you recommend? -- Barbara B.

A: I used to recommend taking 2,000 to 6,000 mg of vitamin C daily (divided into three doses). However, I changed my recommendation in 1999 to 200 mg daily after examining two well-designed studies showing that lower levels of vitamin C more than saturate the body's tissues, and thus are sufficient to protect against cancer, heart disease and other chronic illnesses.
One of the studies that influenced my decision was a review of clinical trials published in the April 21, 1999, issue of the Journal of the American Medical Association.

It concluded that 200 mg a day is the maximum amount of vitamin C that human cells can absorb, making higher doses a waste. The second study came from the Linus Pauling Institute (Pauling himself took 18,000 mg of vitamin C per day) and was published in the June 1999 issue of the American Journal of Clinical Nutrition. It identified a similar dose, 120 to 200 mg, as the optimal amount for reducing the risk of cardiovascular disease, cancer, cataracts and other chronic conditions.

I wouldn't worry if you've been taking higher doses. Vitamin C is water soluble and anything not used by the body quickly passes out. In fact, I still recommend higher dosages - up to 1,000 mg a day if you are coming down with a cold.

The rest of my antioxidant recommendations are as follows:
Vitamin E: 400 IUs of mixed natural tocopherols (or 80 mg of mixed tocopherols and tocotrienols) a day. Since vitamin E is fat soluble, it must be taken with food to be absorbed. Also, choose natural forms of vitamin E (d-alpha tocopherol with mixed tocopherols, or better yet, mixed tocopherols and tocotrienols) instead of the synthetic form (dl-alpha-tocopherol).
Selenium: 200 micrograms a day of a yeast-bound form. Selenium is a trace mineral with antioxidant and anticancer properties.

Selenium and vitamin E facilitate each other's absorption, so take them together. Doses of selenium above 400 micrograms a day may not be healthy.

Mixed carotenes: 10, 000 - 15,000 IUs a day of beta carotene with other carotenoids, such as alpha carotene, astaxanthin, and zeaxanthin. Make sure the mix gives you lycopene, the red pigment in tomatoes that helps prevent prostate cancer, and lutein, which protects against cataracts and macular degeneration.

Q: I recently learned about a nutritional supplement called OPC-3 which is supposed to have benefits for people with allergies and asthma. What are your thoughts about adding this to my two-year old child's diet? -- Raye


A: OPC stands for oligomeric proanthocyanidins, a group of powerful antioxidant compounds. They are commonly found in grape seed extract, which in lab and animal studies has been shown to make blood vessels more elastic and less likely to leak fluids, thus decreasing the leg swelling often associated with varicose veins. OPC-3, a product containing these compounds, is being widely promoted on the Internet as a treatment for diseases associated with free radical damage. However, I have seen no studies suggesting that OPC is an effective treatment for allergies or asthma.


(Grape seed extract is commonly prescribed in France for varicose veins and other vascular problems. I generally recommend it along with horse-chestnut seed extract [HCSE], a more extensively studied treatment for these conditions.)


Asthma is a chronic inflammatory condition characterized by repeated episodes of airway obstruction and associated with an exaggerated airway response to a variety of inhaled stimuli. Although the cause is unknown, such factors as allergies, increased stress, respiratory infections, and exercise can worsen asthma. It can also occur without any obvious triggers.


Allergies are due to "misplaced immunity" against something that really is not a threat to health. Allergic responses are also learned responses that can be unlearned. The goal of treatment should be to convince the immune system that it can coexist peacefully with the substances it is reacting to. In young children, it is important to try to limit exposure to potential irritants of the immune system.


Instead of trying products like OPC-3, my principal recommendation for patients with asthma and allergies (or both) would be to follow an anti-inflammatory diet. Increase organic fruits and vegetables, and stay hydrated with lots of water. Avoid processed foods, partially hydrogenated oils, white sugar, and flour. I also recommend using standard medical treatment selectively and trying to avoid suppressive medications (such as oral steroids) as much as possible. Use mind-body medicine such as guided imagery or clinical hypnosis and experiment with homeopathy as well since children respond favorably to this form of treatment.

Q: I have been using Zyflamend for joint pain for a couple of years because it contains naturally occurring ingredients, albeit in higher concentrations than can be found in their original sources. My question concerns the advisability of COX-2 inhibition in general (in light of the heart problems linked to Vioxx). Is there a risk in continuing to take Zyflamend? -- Anonymous

A: Since the arthritis drug Vioxx was found to endanger the heart and was removed from the market in the fall of 2004, I've often been asked whether Zyflamend, a natural anti-inflammatory product I frequently recommend for joint pain and stiffness, presents a similar threat. Zyflamend, from New Chapter Inc., contains ginger, turmeric, green tea and other herbs that moderate the inflammation associated with arthritis.

One of its components, by the way, is resveratrol, a component of red wine believed responsible for its heart protective effects.

It is true that Zyflamend has some of the same effects as Vioxx in that it inhibits an enzyme (called COX-2) responsible for the joint inflammation that causes arthritis pain. Vioxx and similar prescription drugs described as "selective" COX-2 inhibitors were formulated to target the COX-2 enzyme alone. I believe the herbs in Zyflamend deliver a balanced and proportionally safe effect on related enzymes that were unaffected by pharmaceutical COX-2 inhibitors. Vioxx may have led to an imbalance in enzymes that might have contributed to adverse effects among users.

Zyflamend also contains two other components that may protect rather than endanger the heart. One, holy basil (Ocimum sanctum), contains compounds that have antioxidant effects and may combat high cholesterol and plaque formation. A University of Chicago study concluded that another Zyflamend component, the Chinese herb Scutellaria baicalensis, may also have heart-protective effects.

All told, I see no reason for concern that taking Zyflamend would expose you to the same risks as Vioxx and other pharmaceutical COX-2 inhibitors. The herbs it contains have been used safely for thousands of years by many millions of people. Zyflamend is a supercritical extract of botanicals - meaning the extracts were made with liquid carbon dioxide rather than a solvent such as hexane.

Despite the use of a better, safer solvent, keep in mind that extracts of plants are still not as safe as the plants themselves. Use caution whenever you take any herbal product, especially a concentrated one. As with any product, report any unusual symptoms to your health professional and get checked after a month or two of use to make sure you are getting the desired effect.
